TO: STARFLEET COMMAND
FROM: JAMES DAVIS, COMMANDER, XO, STARBASE 146
SUBJ: DFC RECOMMENDATION FOR WAGNER AND JERACK
CC: COMMANDER, PERSONNEL, STARFLEET COMMAND; FLEET COMMANDER, 146TH FLEET; COMMANDER, STARBASE 146
Sir-
1) On this stardate, the U.S.S. Black River, a Danube-class runabout, was en route from Starbase 146 to Deep Space Nine. The vessel carried two crew and four passengers. The crew consisted of pilot Kurt Wagner, Lieutenant, and copilot Jerrack, Ensign. Passengers consisted of Logica, Captain, U.S.S. Avalon; Fearos M'ssatt, Lieutenant Commander, U.S.S. Avalon; Sareleme Alak'nemu, Lieutenant Commander, U.S.S. Avalon; and Aira, Lieutenant, U.S.S. Avalon.
2) At the time of this incident, Starbase 146's long range sensors were inoperable, with maintenance crews attempting to repair them. U.S.S. Avalon transmitted sensor data to Starbase 146 in order to maintain some amount of observation abilities.
3) During transit, U.S.S. Black River flew directly into an unknown asteroid pack.
4) According to Flight Telemetry, U.S.S. Black River was traveling at to high a velocity to avoid entering the asteroid pack.
5) According to reports from the survivors, LT Wagner and ENS Jerrack attempted to employ evasive maneuvers to avoid the asteroids once forced from Warp. Flight Recorder logs confirm this.
6) After sustaining several impacts, U.S.S. Black River was forced into a crash landing on a large asteroid.
7) LT Wagner and ENS Jerrack managed to employ a skid landing, keeping the vessel relatively intact. LT Wagner and ENS Jerrack were killed upon impact.
8) LT Wagner and ENS Jerrack's actions saved the four passengers onboard U.S.S. Black River. Without their piloting efforts, all personnel aboard U.S.S. Black River would have perished.
9) It is recommend that both LT Wagner and ENS Jerrack receive the Distiguished Flying Cross for their efforts in the preservation of their passengers and for their extrodinary efforts in an impossible circumstance.
James F. Davis, CMDR
Executive Officer, Starbase 146.
